Submitting an Explanation of Benefits (EOB) form

Qualifying EOBs must be:

• An Explanation of Benefits (EOB) form from a hospital based service dated between July 2007 to May 2008.
• From Holmes Regional Medical Center, Cape Canaveral Medical Center, Palm Bay Medical Center, Parrish Medical Center, Indian River Medical Center, Halifax Medical Center
• From one of the following insurance carriers: Cigna, Blue Cross Blue Shield, United Health Care, Health First Health Plans, Aetna

To find out if your Explanation of Benefit statement qualifies for a $100 gas card, please provide the information below. A reminder, all EOBs will be considered for eligibility by category and in the order in which they are received. Only the first qualifying EOB in each category will receive a $100 gas card, up to a maximum of three gas cards per person. Personal information should be removed (blacked-out) from the EOB form before submitting to Wuesthoff.
